CITY OF LYNN COMMUNITY RESOURCE GUIDE
Lynn

Welcome

Welcome To The City of Lynn's Community Resources Guide

The integration and coordination of the delivery of early education and social services to the children and families of Lynn is one of the City’s highest priorities. The vision of the Coordinated Family Community Engagement (CFCE) Council  is for the youth and families of Lynn to live in a nurturing community that respects the unique situation of each child and family. This is accomplished by tailoring services to meet their diverse needs through a seamless system of supports including public, private, community, health and faith-based programs.

In the fall of 2010, Lynn Public Schools, as the lead agent for the CFCE grant, was awarded a Strategic Planning Incentive Grant by the Massachusetts Department of Early Education and Care. The goal of the grant is to develop a plan that leads to the creation of a family-centered system that provides easy entry, clear navigation and appropriate supports for all families. The strategic plan represents an exciting and important commitment to build a more coordinated community and family engagement system for the City’s families. This service guide is only part of what the CFCE Council is putting forward to assist families in Lynn.

We invite you to list your Lynn-based Community Resource on our website. See the second tab on this page to either submit your information online or send an email with the info below. There is no charge to list your service on the City of Lynn website.

Services Elder Service Plan of the North Shore is a non-profit that administers a PACE p rogram. Headquarted in Lynn, we have provided health and home care services to nearly 8,000 frail elders since our founding in 1995. Although our clients are 55+ the impact of our services is experienced by the thousands of caregivers and family members who say time and again, “I don’t know what I would have done without PACE.” Who is eligible for PACE? Those 55 years of age or older who reside in a PACE service area who: • can live safely in the community, as determined by the PACE organization’s Interdisciplinary Team. • during the enrollment process are screened by a nurse as meeting nursing facility clinical eligibility criteria. Upon enrollment participants must receive all health care, primary care and specialist physician services--other than emergency services--as authorized by PACE, or be fully and personally liable for costs of unauthorized services. A pioneer and leader in outpatient addiction medicine, CleanSlate is a multi-state medical group that provides effective treatment for the chronic disease of addiction, primarily alcohol and opioid use disorders.

At CleanSlate, we understand that a one-size-fits-all approach is not effective, so we work with our patients to develop the personalized, flexible, and innovative treatment plan they deserve. We partner with local care providers to ensure patients have access to all the resources they need, including supportive counseling and clear accountability.

We treat people with respect and compassion at CleanSlate. We know our patients did not choose to suffer from addiction, and we are passionate about providing effective treatment that will allow them to achieve the lives they want and deserve – a life filled with hope.

Services CleanSlate Addiction Treatment Centers has been at the forefront of science- and medicine-based addiction treatment since 2009.

As part of Catholic Charities of the Archdiocese of Boston, together our agency serves over 200,000 people throughout Eastern Massachusetts.

The programs at Catholic Charities North strive to prevent hunger and homelessness for local families in a financial emergency; engage youth in education and employment programs focused on long term solutions to poverty; and support families in their effort to build safe, nurturing environments for children.

Services RAW’s continuum of FREE arts-based groups for children and teens aged 6-19 and its young alumni integrate leade! rship development, mentoring, life and work skills, entrepreneurship, teen employment and college access. Group options include the visual and expressive arts, an award-winning film school, single-gender groups, summer camps and more with each group serving between 12 and 17 youth.

Rooted in art therapy, with groups led by art therapists and/or teaching artists, RAW employs the arts as a powerful tool to deeply engage at-risk youth. RAW empowers and supports kids as they explore their identity, overcome their challenges, envision a positive future and develop a plan for how to get there – all through the arts. RAW serves over 430 youth each year in weekly groups and another 850+ in community programs such as Van Go!, RAW’s mobile van that brings art-making to some of Lynn’s most distressed neighborhoods in the summer months.

RAW’s continuum of service to children, teens and young adults offers increased challenge and responsibility at each developmental stage. The continuum culminates with Project Launch, a college access program where volunteer mentors and RAW staff support students and their families through the college and financial aid application process.

Services Our youth mentoring program provides a consistent, structured,one-to-one ! relationship between a youth between the ages of 7 to 18 and a caring, supportive volunteer adult mentor. For a minimum of two hours per week for at least one year, mentors pick their mentees up at their hom and participate in some activity in or around the community. These activities will vary for each match but some examples are a walk on the beach, taking in a new movie, going to the library to pick out a book, strolling through a farmer\'s market, kayaking, do some window shopping at the mall, or sit at a picnic table and eat an ice cream cone and talk. A successful mentor is a trusted friend who provides access to people, places and things outside the mentee\'s routine environment.

Catholic Charities North began more than 95 years ago with initial sites in Salem and Lynn. Through the decades, our programs have evolved to meet the changing needs of our communities. TODAY, we serve more than 18,000 children, youth, and adults annually in communities north of Boston.

Amirah’s Community Resource Center (CRC) is a safe place of support for those who have experienced exploitation.
The CRC is located in a walkable communal location. We work to end the cycle of exploitation through free and accessible services, advocacy and community education. The CRC offers case management, peer mentorship, clinical therapy, group programs, on-site childcare and healing activities for women who have experienced exploitation. We believe that every survivor should have the resources, support, opportunities and community to heal, grow and work towards achieving personal goals.
